LIFE St. Mary Home Care Nurse - Full Time Case Management in Community
**Employment Type:** Full time **Shift:** **Description:** The Registered Nurse (RN) is responsible for providing oversight and management to the provision of service delivery to participants. Provide skilled nursing care, prescribe treatments, and develop appropriate plans of care to participants according to professional nursing standards and primary care orders. Location - LIFE St Mary Hours : 40 hours Shift: 1st shift **Sign On Bonus** $5,000 LIFE St. Mary’s Is seeking a dedicated, experienced, and organized Registered Nurse to join our team. This is a full-time position Monday-Friday with on call weekends every 6 weeks with available overtime pay. Candidate must be able to work autonomously and as an integral part of the Interdisciplinary Team. In this position you will manage a caseload of participants in the Bucks and Montgomery County areas, providing skilled nursing care either in the participant’s homes or in our health center. In addition, the RN will manage and coordinate participant care needs with the IDT, develop and implement individualized care plans, conduct regulatory assessments, and provide ongoing care management to ensure the best possible outcomes. This position does not require Oasis documentation. BSN preferred. Active CPR Certification required. + Minimum of two (2) years of demonstrated successful experience in related services with geriatric population. + Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring. + Valid driver's license in good standing and proof of insurance required.
