Position Purpose:
As a Lead Personal Care Assistant/Worker at Trinity Health PACE, you will play a vital role in delivering compassionate care and support to our participants. You will assist with personal care needs, support health management, and collaborate with team members to maintain a high standard of service. This role also involves aiding the department manager with scheduling and guiding new colleagues.

What You Will Do:

Provide personal care assistance, including toileting, grooming, and mobility support.Assist with nursing care and ensure participant safety by identifying hazards.Engage in activities such as conversation, recreation, and meal serving.Support fellow staff and participate in interdisciplinary team assessments.Maintain accurate documentation of services and participant progress.Participate in ongoing training and adhere to policies, regulations, and ethical standards.

Minimum Qualifications:

High school diploma or equivalent; CNA certification preferred.At least one year of experience with frail or elderly populations, or willingness to undergo training.Proficiency with Microsoft Office and strong communication skills.Ability to work independently, adapt to change, and demonstrate organizational agility.Must meet state health standards and have a valid Employee Medical Statement.
