Are you a passionate Licensed Mental Health Clinician interested in making a difference in the lives of individuals and families at risk of homelessness? We’d love to connect with you about this rewarding opportunity to add value to communities in Fresno, CA.

About the Role: This is a unique, 6-month assignment with the mission to provide wraparound behavioral health services in various affordable housing communities that offer supportive housing services. You would be tasked with helping individuals and families at these brand-new communities with continuous care, from assessment to case management.

Your purpose and mission in this role would be key to the continuous fight against homelessness. Your valuable experience and knowledge would make a big impact in the lives of people going through a hard time and needing a helping hand to get back on their feet.

Key Responsibilities:

Conduct CalAIM assessments Perform diagnosis and documentation Develop and implement treatment plans Maintain progress notes, including details on homelessness and related barriers Engage in field-based work with tenants Connect with unhoused individuals not yet linked to services Complete WIC 5150 applications for psychiatric holds when necessary Provide therapeutic case management Link clients to resources and assist with necessary documentation Offer psychotherapy both in-person and via telehealth Support case management with transportation Attend meetings on housing-related topics Stay informed about housing legislation Build and maintain relationships with property managers Be present in community living spaces, engaging with tenants

Required Experience/Education:

Licensure: Valid license as a Clinical Social Worker, Professional Clinical Counselor, or Marriage and Family Therapist issued by the State of California, Department of Consumer Affairs, Board of Behavioral Sciences. Driver’s License: Valid Class “C” driver’s license or equivalent. National Provider Identifier (NPI): Must have a valid NPI number before starting employment and maintain it throughout your tenure. Credentialing: Must pass initial credentialing and re-credentialing requirements and maintain compliance with state licensing credentialing requirements.

Assignment Details:

Duration: 6-month assignment with potential for stability and prioritization for a permanent/internal position. Schedule: Monday – Friday, 8am-5pm, with a 1-hour lunch break. Location: Split time between 3 housing communities, with possible travel between communities during shifts. Mileage reimbursement at $0.67 per mile.
