GENERAL DUTIES

POSITION SUMMARY: The Social Worker position is charged with serving as part of an interdisciplinary team to provide services to patients living with HIV. Specific social work services include patient assessment, counseling, screening for eligibility of grant-funded services, crisis intervention, facilitating access to medication, and referring to community resources. JOB SPECIFICATIONS:

Education: Minimum Required: Master's Degree in social work. Preferred: Experience: Minimum Required: Preferred: One (1) year of hospital social work experience.

License/Certification: Minimum Required: LMSW

Preferred: Heart Saver or Basic Life Support, Recommended Special Skills/Training:

Minimum Required: The incumbent for this position needs excellent verbal and written communication skills; strong interpersonal skills; an ability to work within a multidisciplinary team; and strong leadership and organizational skills. Other requirements are knowledge of community resources and an ability to assess and intervene appropriately to enhance patients’ quality of life and maintenance of health. Incumbent will also be required to work toward licensure as a Board Certified Social Worker (LMSW). Preferred: Experience with Electronic Health Records (EMR)
