Bolster Heights is seeking an LPN to join our team!This is a full-time 40-hours, benefits eligible position.Starting pay is $27.00 plus a Sign-On Bonus of $7500.

This is an LPN position with our Bolster Heights Long Term Care Facility in Auburn, Maine. The LPN provides professional nursing services to residents in nursing homes and skilled rehab settings, ensuring residents' needs are met through monitoring residents for changes and ensuring safe and quality care. The LPN contributes to the individualized direct resident care to the geriatric resident populations, in conjunction with physicians and multidisciplinary team members.

Education and Experience:

Must possess a current, unencumbered, active license to practice as an LPN in the State of Maine.Current Basic Life Support (BLS) certification required within 3 months of hire.Must be proficient with basic computer skills.

Essential Duties:

Participate in bedside handoff, reporting to nurses coming on duty while including the resident to provide necessary information and facilitate the delivery of excellent resident care.Respond to any issues on the floor during the shift to include but not limited to; staffing issues/call-outs, person supported dissatisfaction, emergency medical situations, etc.Supervise and monitor direct care provided by CNA staff.Communicate with physicians, guardians and other healthcare providers on behalf of the people supported regarding any health changes or concerns.Monitor nursing needs of people supported.Contribute to nursing assessments and care planning.Adhere to Administer medications and treatments as ordered by the physician; coordinate with the pharmacy to arrange medication refills and deliveries. Follow Infection Prevention and Control practices and protocols.Identify resident problems and emergency situations and initiate emergency care and “lifesaving” measures per BLS and other protocols and policies.Act as a resource for, assists and directs C.N.A. staff in ensuring safe, quality resident care is provided.Administer and document resident care, medications and treatments per physicians’ orders within the EMR in accordance with policy.Notify registered nurse and other personnel as necessary, of changes in residents’ conditions and take appropriate follow-up action.Assure housekeeping tasks are completed on unit in accordance with policy. Maintain clean resident rooms, restock as needed and change bed linen according to hospital practice.Ensure documentation meets current regulations and policies.Answer telephone/intercom professionally and communicate information to appropriate personnel.Communicate appropriately and clearly within the interdisciplinary team, including managers, supervisors, co-workers, and providers.Treat residents and families with respect and dignity, identifying and addressing psychosocial, cultural, ethnic and religious/spiritual needs of residents and family.Interact professionally with providers, staff, residents and families.Promote the mission, vision and values of the organization.Maintain a good working relationship both within the department and within the organization.Interpret existing policies and procedures to C.N.A.’s, residents, families and physicians when necessary.Completes mandatory in-services, on-line modules, and competency-based trainings routinely and annually.Follows all employee health procedures, incident reporting, and infection control requirements at all times, as appropriate to the position.
