Responsibilities:

Exercise sound nursing judgment and competency in patient care. Plan, implement, and evaluate nursing care with a patient-centered approach. Communicate with RNs and the healthcare team when patient needs exceed LPN scope. Seek guidance and supervision for new or unfamiliar procedures. Adhere to UPMC policies, LPN Practice Guidelines, and State Practice Act. Set daily patient goals and provide holistic care in partnership with patients. Document patient information comprehensively for clear communication among caregivers. Educate patients and families on care plans and transitions. Support and mentor new LPNs and patient care staff. Actively participate in quality improvement, teamwork, and ongoing professional development.
