Provides leadership in achieving standards of care, collaborating with physician providers and other team members to assess, plan, implement and evaluate individualized patient care in accordance with the current policies and procedures. Chronic wound care experience, preferred. + **Licensure/Certification:** Appropriate state LPN license, required.
