Job Profile Summary 

​This role focuses on providing professional and nonprofessional nursing care services in accordance with physician orders.  In addition, this role focuses on performing the following duties: Functions as part of the health care team in providing safe and effective therapy for assigned patients under the direct supervision of a registered nurse. Participates in the implementation and evaluation of patient care. Requires an LPN license.  A professional individual contributor role that may direct the work of other lower level professionals or manage processes and programs.  The majority of time is spent overseeing the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills typically acquired through advanced education.   An entry level role that applies broad theoretical job knowledge typically obtained through advanced education.  May require the following proficiency:  work is closely supervised, problems faced are not typically difficult or complex, and explains facts, policies and practices related to job area. 

 

Job Overview 

This position works closely with a broad spectrum of healthcare providers and administration to ensure optimal patient care. Along with the patients work with radiologists, primary care physicians, treatment nurses, nutritionists, social workers, community health programs, to ensure each patient receive seamless care.  Responsible for planning, and coordinating a plan of care for patients as part of the multidisciplinary team.  This process would include the coordination of patient appointments and schedules, patient information and follow-up and delivery and oversight of patient care utilizing the nursing process and adhering to the standards of nursing practice. This position follows the patient from one department to another to ensure smooth coordination and patient outcomes. 

 

Job Description 

Minimum Qualifications: 

1. High school diploma or equivalent. 

2. Graduation from an accredited practical nursing program. 

3. Licensed Practical Nurse (LPN). 

4. Two (2) years of relevant nursing experience. 

 

Preferred Qualifications: 

1. Three (3) years of relevant nursing experience. 

 

Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list.  Other duties and responsibilities may be assigned. 

 

1. Utilizes the nursing process for the delivery and management of patient’s experience through the continuum of care demonstrating diagnostic reasoning and critical thinking to ensure a seamless transition for the patient and family.   

2. Informs patient and family of services at hospital, provides information on disease process, tests and the schedule, hours of operation, cancer and related services, contact numbers to call, and provides patient education materials.   

3. Works with physicians and their staff to coordinate patient schedules thru the system. Triages phone calls from patients to assist them and their families in getting services and answers to their care. 

4. Track patients throughout the continuum of care.  When needed follows up with patients to check up on their status and to refer them to resources  

5. Consult and refer patients to other resources such as the LCSW, Oncology Nurse, Dietician. 

6. Serves as a patient and family advocate. Involves patient, family/significant others and other disciplines in decision making related to the patient’s plan of care. 

7. Implements interventions consistent with hospital standards of practice and prescribed medical plan of care. 

8. Documents the plan of care and meets all unit documentation and completes all patient charges promptly and correctly.  

9. Provide care appropriate to the age of patients served in the clinical areas which will include all of the areas that oncology patients usually go.  It may require observation of surgical procedures, procedures in ambulatory settings and radiation medicine. 

10. Explain the purpose and the side effects of drugs and monitoring the patient’s response. 

11. Remains knowledgeable about equipment and utilizes equipment in a safe and cost effective manner. 

12. Collaborates with nursing team members, departments and physicians to ensure the smooth transition and handoff of patients between departments. 

13. Coordinate the appointments, workup, results of tests and other data necessary for the Multidisciplinary conferences. 

14. Maintains clinical information and data as required. 

15. Delegate’s responsibilities to others with an understanding of their roles, knowledge and capabilities.  

16. Facilitates cooperative and collaborative relationships among the various disciplines and departments to ensure effective quality patient care delivery and continuous flow of the patient’s plan of care between all disciplines. 

17. Maintains a collaborative work relationship with peers and colleagues in order to create a positive work environment.   

18. Holds self-accountable for professional practice. 

19. Presents self in a calm and professional manner and effectively verbalizes relevant problems during daily interactions, and/or staff meetings. 

20. Participates in MQSA quality data gathering and analysis for required regulatory framework to support and improve patient care. 

 

Physical Requirements:  

1. Prolonged, extensive, or considerable standing/walking. 

2. Lifts, positions, pushes and/or transfer patients and equipment. 

3. Considerable reaching, stooping, bending, kneeling, crouching. 

4. Frequent exposure to hazardous chemicals, sick patients, bodily substances, noise and possible exposure to radiation, lasers, electric shock, etc. 

5. Regularly exposed to the risk of bloodborne diseases and other transmissible infections. 

6. Contact with patients under wide variety of circumstances. 

7. Subject to varying and unpredictable situations. 

8. Handle emergency and crisis situations. 

9. Subjected to irregular hours. 

10. May have contact with hazardous materials. 

 

Skills & Abilities: 

1. Ability to effectively communicate with patients, families, physicians, and healthcare team. 

2. Knowledge and application of the nursing process to implement a nursing plan of care. 

3. Possesses and applies the skills and knowledge necessary to provide care to patients throughout the life span, with consideration of aging processes, human development stages and cultural patterns in each step of the care process. 

4. Skills and knowledge to provide care to the age groups of the population served including: Knowledge of growth and development; Ability to interpret age specific data and response to care; and provide age-appropriate communication. 

5. Knowledge of pharmacology/medications in administering drugs and monitoring the patient's response.
