Function as a member of the multi-disciplinary health care team under the direction of the Registered Nurse. Maintain the standards of State Nurse Practice Act, and facility policy and procedure, and the Mission and Philosophy of Community Memorial Hospital. **ESSENTIAL DUTIES AND RESPONSIBILITIES:** + Admission: Assists Charge Nurse with completion of Medical Providers orders on admissions; Assures accurate completion of medication sheets on new admissions. + Care Plans: Assists in the development of care plans on admission; Changes in patient conditions are reported to Charge nurse and assures care plans are updated. + Assessments: Assessments performed when notified of a change in patient condition by any member of the nursing staff and reported to Charge Nurse; Notifies provider of changes in patient’s condition, if requested by Charge Nurse. + Medication Dispense: Notifies Pharmacy of any new admits or changes in med orders; Dispenses oral medications according to orders and charted accordingly; Intravenous, intra-muscular, and subcutaneous medications given according to orders and charted accordingly under the supervision of the RN; Medication errors avoided but reported immediately when discovered; Narcotic count completed and reported in Log under the supervision of Charge Nurse; Inhalation therapy given according to orders and protocol. + Discharges or changes in level of care: Notifies pharmacy of change in patient care level or discharge; Upon discharge, under the supervision of Charge Nurse, orders explained to patient or appropriate caregiver and follow-up appointments and prescriptions arrangements made if necessary; Discontinues monitors, I.V sites prior to discharge; Assures home medications are returned to patient upon discharge. + Treatment/Emergency Room Care: Assists Charge Nurse, if necessary, in treatment needs for outpatients in ER or Treatment Room; Gathers updated admission information regarding insurance and biographical information; Calls in additional help when needed especially relating to code blue or trauma codes; Completes appropriate charting and completes transfer paperwork, when necessary. + Skills: Establishes Intravenous sites utilizing aseptic technique with a success rate of 80%; Places urinary catheter utilizing sterile technique with 90% success rate; Places nasogastric tube as ordered and performs airway suction with success; Maintains Gastric tube feeding sites according to protocol; Dressing changes performed according to medical provider orders; All skills performed under the supervision of RN and allowed under Nurse Practice Act. + Paperwork: Completes documentation on medication, dressing changes or treatments given; Document on acute patient condition as necessary; Document on swing bed patients per protocol or any changes in patient condition; Updates patient Kardex as necessary; Performs outdates checks and equipment checks according to schedule and documents. + Infection control: Records in infection control log any incidents of infectious condition on any patients; Notifies nursing staff of isolation needs and assures appropriate personal protection equipment made available. + Education and staff relationship: Gathers a minimum of 9 hours of continuing education; Brings to nursing staff information received from outside workshops; Encouraged to participate in additional education opportunities including ACLS, PALS, TNCC etc; Supports nursing committees such as ER committee, trauma committee, fall committee, care conference, infection control, etc; Communicates with other members of shift nursing staff as well as other departments. Assist other members of shift if available. **Qualifications** **Education:** Graduate of an accredited school of practical nursing. **Licensure:** ND State Board of Nursing current LPN License. (Licensed to practice as a licensed practical nurse in the state of ND or in a compact state. Graduate Practical Nurse (G.P.N.) must practice under the supervision of a registered nurse while the work authorization to practice is valid.) **Experience:** Previous health care experience preferred but not required. **Skills:** Working knowledge of basic clinical skills in delivery of patient care. Holds current CPR; certification of ACLS is encouraged.
