The McGuire Group has an exciting opportunity for a Full-Time Admissions Screener to join our caring and knowledgeable team supporting our facilities in western New York. The admissions screener is works as a liaison between referral sources; and facilities, making both medical and social assessments, coordinating all admissions and transfers to the facility, working closely with the facility admission coordinator and a team of insurance coordinators and leadership in our admission departments.

The McGuire Group is comprised of 5 skilled nursing and subacute rehab facilities located throughout Western New York which includes five skilled nursing facilities We offer a variety of services including subacute rehab, long-term care, and hospice. We are proud to be part of Living Legends Health including facilities in Absolut Care, Vestra Care and Taconic Health Care.  

ABOUT THE POSITION:
The successful candidate will perform a variety of duties, including but not limited to:

Responsibilities

Responsible for completing admission assessments and recommending patients for long term or subacute care Visit designated hospital assignments daily Knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in carrying out the admissions process Interacts with residents and family members, co-workers, clinical and ancillary staff in a non-judgmental, supportive and calm manner Additional tasks as assigned

Qualifications

Degree in Nursing, Social Work or Physical Therapy preferred Experience in Admissions Screening and long-term care is preferred Good Understanding of Medicare and other Managed Care insurance helpful Must be flexible, professional, and reliable Computer skills including keyboarding and PCC Strong customer service skills Must possess a valid New York State Driver’s License and have transportation available for daily job-related travel
