OverviewThe LPN Assistant Clinical Coordinator aids the Clinical Supervisor in scheduling staff and other functions in the office. This position also prints and reviews for accuracy physician orders as well as schedule staff weekly using patient information system.
Responsibilities Schedules CNA visits for the week in the electronic health record a week in advance.Verifies Physician orders and sends complete for MD signature.Participates in the morning Access call in absence of Clinical Supervisor.Assists/Accepts/Documents with physician verbal orders as necessary.Triage RN/SW calls as necessary.Reviews and tracks lab results, fax to MD and place copy in RN box.Emails information for referrals to Access as necessary.Emails patient information and patient change information to Nurse Triage.Administers PPD or other vaccinations for staff as required.Manages orders for equipment as needed.Orders IV meds/supplies from contracted vendor as required.Assists with clinical issues when the Clinical Supervisor is out of the office.Compiles communication through telephone calls and contacts as requested by the Clinical Supervisor. Assists with triaging nursing related calls and dispatching staff.Provides physicians, patients, family members, team members and other callers with needed information.Informs the Clinical Supervisor of unusual or potentially problematic patient/family issues.Consistently adapts communication, assessment, education, and service provision functions to meet age specific needs of patients and families in accordance with branch guidelines for age specific needs.Demonstrates initiative and skills in planning and organizing work, sets and meets objectives and finds increasingly efficient ways to perform tasks. Requires minimal supervision and is self-directed.Uses own knowledge and experience base and other resources as necessary to make logical decisions and solve problems. Qualifications High School Diploma or GED equivalent requiredAdvanced education for clinical licensure; successful completion of IV therapy course (30 hours)Minimum of 2-3 years nursing experience required, preferably in oncology, palliative care, geriatrics, or home health care.Experience working with interdisciplinary team preferred.Requires a current Cardiopulmonary Resuscitation (CPR) certification (American Heart Association).Current driver's license and insurance.Current, unrestricted LPN license in the state of Florida.Position requires expertise in various technologies such as Microsoft Office, electronic medical records, etc.Demonstrated ability to work non-judgmentally with persons of varied cultural, religious and socioeconomic backgrounds and lifestyles.Percentage of travel: 25-75% About our Line of Business At Haven, our primary ambition is to bring high-quality care to the warmest and most serene environment possible: home.
