Position Highlights & Primary Responsibilities:
In partnership with the Medical Management team and Physician Leadership, will perform case management, coordination of care and medical management consulting within an interdisciplinary team specific to Emergency Department discharges Collaborate with acute and post-acute network facilities, vendors and Optum partners on complex patient care coordination Positions in this function provides health education, coaching and treatment decision support for membersOptum NY/NJ was formed in 2022 by bringing together Riverside Medical Group, CareMount Medical and ProHealth Care. Required Qualifications:
Current unrestricted NY LVN license 2+ years of experience in a physician’s office, clinical or hospital setting Proven knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF and IHD Proficient computer skills to work efficiently with electronic medical recordsPreferred Qualifications:
Experience related to patient education and/or motivational interviewing skills and self-management goal setting Supervisory or team lead/SME experience Case management experience Ability to be comfortable speaking and presenting to hospital partners and executive leadership*All employees working remotely will be required to adhere to UnitedHealth Group’s Telecommuter Policy.
New York Residents Only: The hourly range for this role is $19.47 to $38.08 per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
