Adecco Healthcare & Life Sciences is hiring an LPN Case Manager! This role is remote / work from home and you must reside within California to qualify. Please read below and apply with an updated resume for consideration. **Type:** Three-month contract, opportunity for extension or full-time offer Schedule: Monday through Friday, 8am to 5pm, with a few 10am to 6:30pm shifts Pay: $32.50 to $34.50 an hour **Responsibilities of the LPN** · Contribute to the assessment of health status through gathering and recording of assessment data, observing, and monitoring and reporting of changes in condition to the supervising RN. · Conduct interviews to understand cause and effect, gather or review health history for clinical symptoms, and determine health needs. · Collaborates with internal and external stakeholders on managing a successful transition from hospital to home. · Responsible on completing timely post discharge patient outreaches to identify, develop, implement, and assist on interventions for high-risk patients. · Supports and manages the post-discharge needs to reduce hospital readmission, improve patient care engagement, and improve patient experience. **Qualifications of the LPN** · Active and good standing LPN license in California · Must reside within California · Past experience with CURO, EPIC, Nextgen EMR, MS tools are a plus **Why work for Adecco?** · Weekly Pay · 401(k) Plan · Skills Training · Excellent medical, dental, and vision benefits Benefit offerings include medical, dental, vision, life insurance, short-term disability, additional voluntary benefits, EAP program, commuter benefits and 401K plan.
