Amergis Staffing is seeking a Nurse Case Manager (LPN) to work in the community with adults with severe and persistent mental illnesses. The ideal candidate would possess knowledge of psychiatric medications, psychiatric and medical treatment. This person would be comfortable with outreach, case management and providing support in individual’s homes. Details of the position include: Location: Wakefield, MA Program: ACCS (Adult Community Clinical Services) Schedule: Mon-Fri, 8:30-4:30 Start Date: Two weeks from offer Salary: $48/hr *MUST have a Massachusetts license and reliable transportation as you will be doing home visits for clients. Duties and Responsibilities (these are a general summary and not all inclusive): Act as a primary case manager to 4-8 clients (clients live in surrounding areas) Provide nursing interventions to individuals Assist in the management of medications and the overall functioning of the nursing department Formulate and regularly update Comprehensive Assessments and IAP's for clients on your caseload Actively take part in supervision, staff meetings, in-services and trainings Transport clients to work sites, meetings, daily activities, appointments, and activities as needed. Qualifications - Must have a reliable vehicle - 1 year experience with mental health/psychiatric patients - 1 year of med administration - 1 year knowledge of psychiatric medications, psychiatric and medical treatment *Bilingual a plus!
