Provides nursing services to patients and families in accordance with the LPN scope of practice as defined by the Illinois Board of Nursing. As a Rehabilitation Nurse at Carle Health, you'll play a vital role in helping patients regain their independence and confidence. Working closely with a multidisciplinary team, you will provide expert care that emphasizes physical, psychological, and emotional well-being. Qualifications: EDUCATION REQUIREMENTS Highschool Diploma/GED CERTIFICATION & LICENSURE REQUIREMENTS Licensed Practical Nurse (LPN) Illinois upon hire and Essential Functions: + Administers medication, hangs IV piggyback antibiotics and fluids, excluding IV push and high-risk medication. + Communicates with providers to receive orders and coordinate care for patients. + Charts assessments on assigned patients. + Completes dressing changes as ordered. + Completes trach care as needed. + Manages NG, g-tube, j-tube, including feedings and medication as prescribed. + Cares for and changes ostomy. + Reinforces education and discharge instructions. + Charts on care plans. + Reviews with patient/family medications, treatment and after visit instructions. + Collaborate with providers, therapists, and RNs to develop individualized care plans. + Manages vital signs and provides hands-on nursing care.
