MercyOne Sioux City is seeking a compassionate and skilled Home Health Licensed Practical Nurse (LPN) to join our dedicated healthcare team. The LPN will provide in-home nursing care to patients, supporting their medical needs and promoting their independence. This role involves working closely with patients, families, and interdisciplinary healthcare teams to deliver high-quality, personalized care in the comfort of patients' homes.

Key Responsibilities:

Provide direct patient care under the supervision of a Registered Nurse (RN) or physician, including medication administration, wound care, and monitoring of vital signs.Collaborate with healthcare providers to develop and implement individualized care plans based on patient needs and physician recommendations.Educate patients and their families on disease management, medication administration, and lifestyle modifications to improve overall health outcomes.Perform routine assessments and document patient conditions, treatments, and progress in accordance with healthcare regulations and MercyOne policies.Coordinate with the interdisciplinary team to ensure comprehensive, patient-centered care is delivered.Monitor and report any changes in patients' health status to the supervising RN or physician promptly.Maintain up-to-date knowledge of home health care protocols, infection control practices, and safety guidelines.Ensure proper use of medical equipment, supplies, and documentation software.Comply with regulatory and MercyOne standards for home health care, including participation in quality assurance and performance improvement programs.Assist in coordinating services such as physical therapy, occupational therapy, and social work as needed.

Qualifications:

Active Licensed Practical Nurse (LPN) license in the state of Iowa.Minimum of 1 year of clinical nursing experience, preferably in home health or community-based care.CPR and Basic Life Support (BLS) certification required.Strong interpersonal skills and the ability to work independently in a home setting.Knowledge of home health regulations and documentation practices.Proficient with electronic health records (EHR) systems and basic computer skills.Valid driver’s license and reliable transportation, with willingness to travel to patient homes in the Sioux City area.

Preferred Qualifications:

Experience with wound care, medication management, and chronic disease management.Familiarity with home health equipment and telehealth technology.Experience with geriatric or hospice care.

Physical Requirements:

Ability to lift, carry, and transfer patients as needed.Ability to drive between home visits.Must be able to walk, sit, and stand for extended periods during patient visits.
