Position Purpose:
LPN: Revisit Nurse position provides skilled nursing care to the organization’s clients in accordance with the plan of care as prescribed by the physician, and assigned by the supervising registered nurse.
Monday-Friday, 8a-5:00p, weekend/holiday rotation as required
What You Will Do:
Effectively and efficiently plans visit content and schedule based on client capabilities, condition, preferences (when possible), physician orders and available resources.
Maintains a working knowledge of applicable Federal. State and local laws and regulations, the Organizational Integrity Program, Code of Ethics, as well as agency policies and procedures and adheres in a manner that reflects honest, ethical and professional behavior.
Performs specific treatments and administers medications to implement the medical and nursing care plans.
Applies appropriate preventive, therapeutic, and rehabilitative nursing procedures and techniques to clients.
Utilizes interview, observation and evaluation in assessing clients and applies nursing judgment, consistent with practice standards, in making recommendations to the primary nurse and client.
Minimum Qualifications:
Graduate from an accredited or state approved school of practical nursing.
Current licensure in the state of Connecticut.
Must have a minimum of one (1) year experience in an acute care setting.
Home healthcare experience preferred.
Must possess interpersonal skills sufficient to interact effectively with clients and their families as well as all interdisciplinary caregivers, peers, subordinates and supervisors.
Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
Must have current Driver’s license and reliable transportation to and from work site.
