Schedule: Monday-Friday, 8am-5pm with rotating weekends.

Location: will travel/service around Snohomish County

Job Description:

The Licensed Practical Nurse assists the Registered Nurse in the planning and delivery of nursing care and to assist with the rehabilitation of patients in their place of residence on an intermittent basis.   The RN develops a treatment plan which is instituted in accordance with physician orders.  The LPN also assists in educating patients to promote self-care.

Required Qualifications:

Graduate of an accredited Licensed Practical/Vocational Nurse program Washington Licensed Practical Nurse upon hire National Provider BLS - American Heart Association within 30 days of hire. National Provider Identifier (NPI) number within 30 days of hire. Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information. Preferred Qualification:

1 year recent hospital, nursing home or home health experience
