Responsibilities

Parkview Health at Home Now Hiring LPN Home Health

Mon -Fri (8:00 AM - 5:00 PM) 

No on-call requirement with flexible schedules.  Provides care and educates patients and their caregiversCoordinates services with other disciplines and community resources as requiredEvaluates and updates care plans and assessments in response to patient and environmental changesAttends regular case conferences, in-services, and special case orientations as required, and participates in on-call rotation

Qualifications

Active LPN LicenseOne year of clinical experience as a LPN requiredValid Driver’s LicenseProficient in technology, including smartphones, tablets, virtual reality, interactive software, and artificial intelligenceIn Washington State, all providers must meet the National Provider Identifier (NPI) requirement and provide proof before onboarding.
