The Licensed Practical Nurse (LPN) performs a variety of professional nursing duties in compliance with the Comprehensive Home Care regulations and follows established policies and procedures for our beautiful Assisted Living/Memory Care building (Meadow Woods) in Bloomington, MN.

As part of the Martin Luther Campus Senior Community, Meadow Woods Assisted Living is a residential setting by woodlands, wetlands, mall of america, and the Minnesota River Bluffs. Our 113 unit assisted, independent and memory care buildings are located in East Bloomington, MN near public transportation. Responsibilities/Job Description Provides skilled nursing care such as; implementing practitioners’ orders, ordering and assisting with medications and supplies, operates medical equipment, and charting practicesLifts and transfer residents as needed, Initiates emergency treatment as neededMonitors work and performance of unlicensed staffUpdates RN with current observation and completes documentation of resident health statusCommunicates changes in residents conditions to relevant individualsMay support RN by completing resident assessments, service agreements and care plans QualificationsRequired:
MN LPN licensureAHA BLS for Healthcare Providers or Red Cross CPR for Professional Rescuers(3 week grace period for BLS) Able to work independentlyGood communication and interpersonal skillsPreferred:
