Position Summary:

As a Licensed Practical Nurse (LPN) with Aultman Medical Group you will be responsible for supporting our providers in delivering exceptional patient care.  This includes but is not limited to rooming patients, taking vitals, drawing blood, giving vaccinations, performing EKGs, answering phones, taking messages, sending messages to appropriate receiver, proposing refills, helping patients, providers and coworkers.

 

 

Department Summary:

Aultman Medical Group Family Medicine of Louisville is a Family Medicine practice with at least two providers and multiple supporting colleagues located in Louisville, Ohio.

 

Job Qualifications:

Active License for Practical Nursing in the State of Ohio Medical Office experience preferred

Responsibilities & Expectations:

As a LPN for Aultman Medical Group duties will include, but are not limited to:

Performs general nursing care to patients.  Administers prescribed medications and treatments in accordance with nursing standards and provider orders. Assists with the preparation of equipment, and aids provider during treatment, examination, and testing of patients. Observes, records, and reports patient’s condition and reaction to drugs and treatments to providers.  Administers prescribed medications as directed. Prepares charts in advance gathering all outside consults/diagnostic testing/lab work ordered and performed since last visit. Prints Enhanced Encounter Forms Identifies Medicare Wellness patients. Monitors Outlook emails daily Manages Work Queue manager by scanning and indexing outside documents. Completes all outstanding items in multi patient task list Assists in the completion of all quality initiatives Aids in the amendment of all incomplete or incorrect billing errors Read and document TB test results Administer controlled medications Administer IV therapy Oversees appointment bookings and ensures preferences are given to patients in emergency situations.  Maintains timely flow of patients. Greets patients and prepares them for provider examination.  Screens patients for appropriate information.  Instruct patients in collection of samples and tests. Completes patient intake, reviews and updates medical history and medications. Verifies patient pharmacy. Gives injections and applies dressings as required. Obtains and runs testing as ordered (urine dip, urine micro, glucose, A1C, EKG,PT/INR) Obtain pt consent for procedures Performs nurse visits- including education of physician’s orders to patient and proper charting after visit. Arranges for patient testing and admissions. Schedules STAT testing (as needed) Responds to and refers incoming telephone calls.  Instructs patient and family regarding medication and treatment instructions.  Formulates and updates patient care plans.  Orders, prepares, and inspects patient’s charts.  Posts test and examination results. Responsible for preparation of exam rooms including re-stocking of treatment areas. Identifies clinical and office supplies and equipment needed and notifies lead and/or practice manager. Attends required meetings and participates in committees as requested. Participates in professional development activities and maintains professional affiliations. Maintains patient confidentiality. Performs related work as required per department.  (See required duties specific to department.) Comply with all statutory and regulatory policies and procedures. Teaches patient education. Assist provider with PAP testing or I&D’s Suture and Staple removal Processes prior authorizations for testing and medications Updates health maintenance Double checks medications and immunizations. Reviews CMA with patient and runs urine drug screen Check to ensure CMA pts have a current med contract and appt scheduled.  Once refill is completed, ensure note contains providers OARRS statement.  If not, return to the provider to complete. Drops appropriate quality codes Maintains patient files, records, and other information. Obtains information from paper charts or Allscripts (as needed) Requests records from outside offices Prints Primetime EE and Optum forms Monitors Clinical Pool and personal inbox Files incoming faxes, prints fax orders for physician signature, transcribes outside labs and INR results.  Faxes signed orders and scans into workque to be filed. Uploads Primetime EE’s to Aultcare Portal Works the outstanding order list (closes the loop) Cross trained to check in/check out patients, make appointments, work front desk. Additional duties as needed

 

 

 

Working Conditions:

Work eight to ten hours a day. Lunch periods and breaks are coordinated with other colleagues to maintain adequate floor coverage Cross training in all areas in the department is required. Staff may be assigned to different locations within AMG/ANCMG

Physical Addendum:

 

Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye-hand coordination.  Requires sitting, standing, and walking for extensive periods of time.  Requires occasional lifting and carrying of items weighing up to 100 pounds.  Requires corrected vision and hearing to normal range.  Requires working under stressful conditions or working irregular hours.  Requires exposure to communicable diseases or bodily fluids.
