Graduate of an accredited LPN/LVN school of nursing National Provider BLS - American Heart Association within 30 days of hire Oregon Licensed Practical Nurse upon hire Washington Licensed Practical Nurse upon hire - if practicing within this state Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request.
