Job Summary:
Responsible, under the direction or delegation of a Registered Nurse as a member of a multi-disciplinary team, to provide quality practical nursing care consistent with the scope of licensed practice and the standards of care. Employee will perform practical nursing care tasks, carry out routine aspects of the designated nursing regimen, participate in patient care planning, assist with and/or perform therapeutic and diagnostic procedures, report/document observations of patients, and assist in maintaining an optimum physical and emotional environment for assigned patients.
Essential Duties:
Observes, collects, and records data related to patient’s biophysical, psychosocial, environmental, and self-care ability, as well as educational and discharge planning needs.Follows an age appropriate plan of care in conjunction with the physician, patient, patient’s family and/or significant other, and other members of the interdisciplinary health care team.Documents and utilizes patient data to assist in measuring progress toward attainment of patient outcomes and patient goals.Effectively manages and utilizes sophisticated equipment associated with the floor.Involves patient and significant other in education and decision making process related to patient’s care.Identifies the learning needs of the patient and/or significant other and initiates measures to meet those needs within the patient’s assessed learning abilities.Reiterate and reinforce education, originally given by a physician or RN regarding medications, pre & post-surgical information and discharge needs.Administer those medications for which they have the needed knowledge, skills, and demonstrated competency to proceed safely. If a comprehensive assessment of the client condition is needed before medication administration, the RN or licensed independent practitioner must complete the assessment and direct the involvement of the LPN.Provides for and follows precautions to ensure patient safety related to fall prevention.Ensures that equipment is available, in proper place and maintained in working condition.Qualifications
License/Certifications:
Current license as an LPN issued by the State of Oregon required. Must have current American Heart Association BLS for Health Care Providers within 30 days of hire.Experience:
Must have communication and interpersonal skills in relating to public and peers.
