**Job Description:**
Seeking a Licensed Practical Nurse or Trained Medication Aide to serve at Grace Home, our long-term care facility in Graceville, Minnesota. This LPN or TMA works on a casual, as-needed basis. Some holidays and weekends are required.

**LPN ($24.67-$37.01 based on experience):**
A care giver who assumes responsibility and accountability for a group of primarily elderly patients for a designated time frame and provides care to these patients via therapeutic use of self, the nursing process, the environment/instrumentation, and other health care team members. Works both cooperatively and independently, to work efficiently under stress.
**Medication Aide ($18.59-$27.64 based on experience):**
Responsible for providing direct patient care to skilled nursing facility residents and for servicing the needs of residents in a manner conducive to their safety and comfort, within the scope of the nursing assistant role. Provides personal hygiene care, assists with providing for nutritional needs, recognized and deals with the problems of the aging process, recognizes and promptly report changes in resident condition to shift nurses, and maintains a safe, clean, attractive, and neat environment for the residents. Must be aware of and prevent the abuse of vulnerable adults in the care of the Essentia Health skilled nursing facility. Maintains awareness of safety issues and strives to accomplish all responsibilities in a safe manner. Identifies safety hazards and emergency situations and initiates corrective actions. This position also works as a team member in the nursing department, administering medications to residents as ordered by the physician, under direct supervision of the nurse on duty. Other tasks include keeping proper care records, reporting any changes in resident condition, and interacting with internal and external customers in a professional manger while ensuring resident safety.
**Licensure/Certification Qualifications:**
+ LPN: Current LPN licensure in the state of Minnesota.
+ Medication Aide: Current and in good standing CNA on the Minnesota Nursing Assistant Registry.
+ Medication Aide: Trained Medication Administrator Certification.
+ All: Basic Cardiac Life Support (BCLS) certification within 1 month.
