LPN School Nurse - Sylvania Schools - Part Time

 

The primary focus of the LPN school nurse will be to assess the needs of the student population in the school;  provide direct nursing services such as chronic disease management, immunizations and respiratory treatments, etc. for students requiring these services; work collaboratively with Nurse, Nurse  Practitioner or Physician; provide education to students; train school staff who may be assigned (when appropriate) to care for a student with special health concerns; and implement school health policies and procedures.

 

ACCOUNTABILITIES 

Collaborate with school and school nurse on health care plan and emergency protocol for student requiring accommodations at school. Utilized telehealth effectively. Perform all necessary and applicable medical treatments, point of care testing, and procedures for chronic disease. Completes ongoing health assessments and plan health management protocols for students with chronic health problems. Monitor/support for designated school personnel delivering health services. Monitors immunization rates and compliance. Coordinate mandated health screenings. Direct/provide health screening and follow up including assistance with access to care. Connect students to resources including insurance. Communicates health status of student to staff, parents and health providers. Teach designated faculty/staff how to monitor and respond to emergencies that may occur in children with chronic diseases/special needs. Provide individual classroom education and counseling as needed. Provide appropriate overall health care needs for students in the school. Identify community resources for health related issues. Develop relationships within school district and community to meet health needs. Advocate for student health. Participate in district level school/community groups. Coordinate, link, refer and obtain health services (dental, vision, transportation, access to care) for individual students. Serve as a liaison/provide leadership for health issues in the building. Establish communication and provide consultation services to parents, the community, and all other health care providers. Provide health leadership in school multidisciplinary teams. Provide for ongoing health care education for students, parent/community/employee health promotion and education.
