LPN Team Lead
 

RESPONSIBILITIES:

Lead the team on your assigned unit, assuring completion of all assigned resident care needsProvide direct "hands-on" patient care, including passing medications and performing treatmentsOversee and guide CNA's work to ensure quality resident care and completenessAssists in the admission, transfer & discharge of residentsCommunicate closely with the Supervisor/Manager in terms of resident problems, progress, and care plansDirect and demonstrate individualized supportive care to the residents and their families while working cooperatively as an integral member of the multidisciplinary team

ATTRIBUTES OF SUCCESSFUL LPNs:

Current Licensed Professional Nurse license in the State of New YorkGood organizational and communication skillsRespect for the privacy, dignity, and confidentiality of all residentsPrior experience in long-term care is helpful but not necessary
