Under the direction of a registered nurse, provides nursing care to assigned patients in the home environment.
Responsibilities Follows physician orders and 485 Plan of care. Performs skilled nurse visits per physician orders and completes all assigned tasks as listed on patient pathway and plan of care as appropriate to patient needs.Reviews medications with each patient on each visit to insure compliance with taking meds, and that all meds are taught and that the med profile is accurate and up to date. Performs wound care measurements weekly.Communicates regularly to the RN regarding patient problems & progress toward goals. Notifies physician and follows through on all orders received.Interacts with patient/family and caregivers in a supportive, mature, knowledgeable manner and provides compassionate, safe and organized care.Responsible for truthful, accurate, timely and complete documentation

QualificationsGraduate of a school of nursing, with current Alabama LPN license. Prefer a minimum of 1 year recent skilled nursing experience. Prefer experience in home care. Must be able to read, write legibly, speak, and comprehend English. Must have current BLS certification or be able to obtain within 60 days of employment and before first independent patient visit.
WORKING CONDITIONS
Must be able to lift 50 pounds, and turn and position patients weighing over 200 lbs. Ability to stand, walk, kneel, sit, crouch or stoop with good balance for extended periods of time. Must be able to maneuver over a variety of uneven surfaces including fields, grass, ditches, gravel, concrete, wood, vinyl or carpet. Good manual and finger dexterity. Good communication skills. Must be licensed and insured to operate a motor vehicle. Must have tolerance of variety of environments to include presence of pets and infestations. Physical presence onsite is essential. Hearing and vision must be normal or corrected to within normal range. Able to perform the duties with or without reasonable accommodation.
