Limited Secure Placement (LSP)is part of a groundbreaking initiative to provide services and supports for juvenile justice system involved youth in residential placement facilities closer to their families, communities, and schools. Youth who are referred to our Limited Secure Placement facility through Family Court receive a multitude of services in placement and in the community when they transition to “Aftercare.” Planning for a youth’s release from LSP into the community begins at intake and service planning continues through the end of the youth’s delinquency placement order.
The Family Advocateprovides supportive case management services to youth, ages 12-21, to support them as they transition safely from residential placement back into the community. The role of theLSP Family Advocateis to streamline services, support service plan goals, guide the discharge readiness planning while youth are in placement, and follow through with the youth/family in the application of the service plan in the community.
Responsibilities

+ Build and maintain a supportive relationship with youth and families to maintain their engagement in service planning.
+ Utilize the Risk, Needs & Responsivity framework to identify youth/family protective factors, risk factors and incorporate youth and family input to develop discharge service plan goals and overcome barriers to engagement. Responsible for developing the discharge plans with LSP/PPS team, youth, and family.
+ Monitor and coordinate family and community visits. Supervise the youth and family interaction during facility visits as needed and as determined by Director of Case Planning or Program Director.
+ Participate in LSP facility programming including treatment team meetings, team meetings, recreational and family engagement activities (i.e. Family Team Conferences, Family Night and Release Ceremonies).
+ Together with the Clinician, and Case Planning Team facilitate a family support group to engage caregivers.
+ Beginning 60 days prior to release, the Family Advocate will accompany the LSP Case Planner in monthly home visits. As youth is nearing release, the Family Advocate will emphasize focus on family engagement/discharge readiness. The Family Advocate will accompany youth on home/community passes as determined by the treatment team.
+ Within the first 24-hours of release, the Family Advocate will complete a face to face contact with the youth. They will maintain weekly home/community visits with the youth during the first 6 weeks post-release. Afterwards, the number of required home visits will be determined by the YLS and other safety factors identified by treatment team (including PPS).
+ Responsible for all case planning responsibilities when youth are discharged from facility and until their delinquency docket expires. Support the youth/family in following through with the discharge plan. Provide supportive casework counseling and crisis intervention. Face to face contact with youth to address and asses safety following an incident is expected within 24 hours of incident.
+ Collaborate biweekly with the Placement & Permanency Specialist (PPS) about the youth’s adjustment and progress on aftercare, including any high risk/safety concerns.
+ Maintain biweekly collaboration and communication with all service providers, including school staff, JJI/FFT/MST, Mentors, Therapists, etc. to organize and streamline case planning services and to effectively address safety/risk concerns.
+ Participate in all Close to Home Family Team Conferences.
+ Evaluate the educational history, academic strengths and weaknesses, need for or history of an Individualized Education Plan (IEP), educational goals and preferences for adult life. Ensure youth is placed in appropriate educational setting and support the family in advocating for any changes necessary. Collaborate regularly with community school staff to ensure that program goals and IEP goals align and that youth receive all necessary supports.
+ Coordinate and network with community partners and Educational/Vocational specialist for opportunities for youth/families.
+ Assist youth with successfully completing any probation or aftercare programs mandated by the courts. Identify resources or services that meet the needs and/or court mandates of youth and make referrals as necessary.
+ Maintain clear and concise documentation that includes discharge planning, engagement, assessment, child well-being and progress on Aftercare. Complete all FASPS (when youth is on Aftercare), progress notes, case closing summaries, Plan Amendments, PYA’s, Transition Forms and any other reporting/case management requirements via Connections in a timely and accurate fashion (within 5 business days).
+ Participate in all agency, ACS/CTH and program training and weekly individual supervision.
+ Model reliable, supportive and positive relationships to help youth navigate challenges and achieve their goals. Model professionalism and respectful behavior in both the community and in facility.
Qualifications
Required Education and Experience:
+ Bachelor’s Degree with;
+ A t least two (2) years of documented experience in family engagement and treatment.
+ Preferred experience in working with adolescents involved in Child Welfare/Juvenile Justice systems.
+ Bi-lingual (English/Spanish)-speaking preferred, not required.
Starting Salary $48,000.00/Annually
