The Unaccompanied Children Long Term Foster Care Program provides a safe and secure placement to Unaccompanied Children (UC) who following an initial placement in an ORR/DUCO care provider facility, and after efforts for the safe and timely release indicated that no sponsor is willing or able to care for the UC, and the UC is expected to have a protracted length of time in ORR care. Working in collaboration with the federal Office of Refugee Resettlement, ORR, our program provides a safe and nurturing environment for youth in licensed foster homes in the community. We provide therapeutic, medical and educational services, as well as other supports that youth need to achieve success through an array of strength-based and targeted interventions as we link them with family members and/or other sponsors in the United States. About the Role:
The LTFC Case Manager oversees the minor's case, service delivery, concurrent planning and reunification process. The LTFC Case Manager acts as the liaison between the program and all stakeholders involved in the child’s case. This role also ensures holistic support with the child’s treatment team as the minor continues in the existing placement.
Responsibilities Case management responsibilities include all necessary tasks as required by the Federal Office of Refugee Resettlement. Conduct initial intake assessment and orientation for all new LTFC minors within 24 hours of admission.Upload all trigger reports and admission documents in a timely manner within the UC Portal.Conducts initial and monthly Foster Home visits to assess placement and overall well-being of the youth. Endure close communication with medical coordinator for all child’s medical appointments and referralsDevelops and maintains contact with youth and their service providers for school, medical, social and other related service needs. Establish prompt communication with potential sponsors, complete the family reunification process including required background checks, thoroughly vet and ensure sponsors are a safe discharge placement and resource for the child. Follow up with legal service providers regarding the timely submission of any documentation or correspondence needed for the legal relief process. Performs discharge and release functions to ensure that each youth has a safe, appropriate and prompt release plan. Documents client contacts and other case required paperwork in an accurate and timely manner in accordance with regulations. Ensure all ORR, OCFS, and Education mandates and requirements are met. Maintain a high standard of filing and record-keeping according to ORR policies and procedures. Redact and submit all record requests to ORR. Provide oversight of Fed Ex processing and submission of CA/N check requests to ORR. Sort and file discharge paperwork or paperwork of minors that have been previously discharged. Works collaboratively with the Clinical Team and other program staff across the department and the organization. Some evening and weekend hours. Qualifications Bachelor’s degree in a Human Services field. Prior Case Management experience required.Knowledge of child welfare and immigration related legislation Excellent verbal, written communication and organizational skills are essential Proficient in Microsoft Office suite, knowledge of Connections Bilingual (English/Spanish) a mustSupervisory Responsibility
This position does not have direct supervisory responsibilities. The LTFC Case Manager will work independently with guidance and support from their supervisor and other team members, collaborating closely on case management and service delivery.
Work Environment:
This role requires four in-office days and one remote day per week, with flexibility for after-hours, weekend, and holiday support as needed. The position involves collaboration with multiple teams, fieldwork, and travel for foster home visits to assess for adjustment and safety of the child in their home environment.
Position Type/Expected Hours of Work:
This is a full-time position with an annual salary of $64,537.20. The position operates Monday through Friday, from 9:00 AM to 5:00 PM, with flexibility for after-hours, weekend, and holiday support as needed.
