Position Scope: Administers personal care and assistance with activities of daily living for patients in the home setting under physician’s orders, assignment and supervision provided by the Administrator or Registered Nurse.
Qualifications/Education/Experience:
+ High School Diploma or GED
+ Experience with older adults desired.
Responsibilities:
+ Implement the care plan established by the Registered Nurse, or other Professional.
+ Notify the Registered Nurse/Supervising Nurse of patient’s progress/lack of progress and the need to adjust the care plan.
+ Promptly report changes in patient’s physical and/or mental status.
+ Administer or assist patient with personal care, hygiene and grooming, including bath or shower, hair, skin, mouth, nail care, dressing and undressing
+ Monitor and record vital signs.
+ Assist patient with elimination using bathroom, commode, bedpan, or urinal.
+ Assist patient with safe transfer and ambulation activities.
+ Assist with urinary catheter irrigation and with insertion of rectal suppositories
+ Assist patient with prescribed exercise after instruction by the Registered Nurse or Physical Therapist.
+ Assist with household services that are essential to the patient’s home health care.
+ Observe, report, and document patient status, care or service furnished, submitting activity records daily. Verbal reporting of pertinent observations of patients’ physical and psychosocial status to Registered Nurse, Therapist, or Supervising Nurse when immediate changes are needed.
+ Document accurately and concisely all care and observations.
+ Attend and participate in pertinent in-service education programs.
+ Maintain strict confidentiality.
+ Performs other duties as required.
Physical and Sensory Requirements: (With or without the aid of mechanical devices)
+ Appropriate professional attire as required by Lutheran Home Cottages dress code.
+ Must possess the ability to make independent decisions when circumstances warrant such action.
+ Must possess the ability to deal tactfully with staff, residents, family members, visitors, government agencies, and the general public.
+ Must possess leadership and willingness to work harmoniously with and supervise professional and non-professional staff.
+ Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to relate to residents based on the level at which they are currently functioning.
+ Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing practices.
+ Must be able to move intermittently throughout the workday.
+ Must be able to speak and write the English language in an understandable manner.
+ Must be able to cope with the mental and emotional stress of this position.
+ Must possess sight/hearing senses or use prosthetics that will enable the senses to function adequately so that the requirements of this position can be fully met.
+ Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, staff and support agencies.
+ Must be in good general health and demonstrate emotional stability.
+ Must be able to relate to and work with ill, disabled, elderly, emotionally upset and at times, angry people within the facility.
+ Must be able to lift, push, pull, and move a minimum of twenty-five (25) pounds.
+ Be required to work up to 8 hours of overtime as staffing changes may occur on a shift to shift basis
+ Must be able to assist in the evacuation of residents.
