Job Description:

The Licensed Vocational Nurse (LVN) Coordinator is responsible for providing coordination and clerical support to the Home Health field clinicians and operations team on site in our Santa Barbara office. Major responsibilities include facilitating and supporting communication with team, support of proper documentation, participating in chart audits and order review.
Additional responsibilities include obtaining information to continuously improve the department’s performance in process measures. Supervision is provided by the Director, Clinical Managers or RN designee. Respects and promotes the mission, vision and values of VNA Health.

Job Duties:

Responds to patient and field staff requests for information and support as needed, as received through various channels of communication.Assists department in coordinating team communication channels.Conduct regular audits of patient records, including reviewing face-to-face documents and chart entries, to ensure compliance with state, federal, and accreditation standards. This involves verifying the accuracy and completeness of documentation, ensuring all clinical information aligns with VNA Health's standards, and tracking key process measures such as medication follow-up, heart failure follow-up, and vaccine administration to support quality improvement initiatives.Facilitate communication with physicians and the Wound Care Center to clarify and resolve any discrepancies in patient orders, ensuring accurate and timely care.Transcribe these orders into the electronic medical records system with precision, maintaining consistency across patient care plans and avoiding errors that could impact patient outcomes.Prepare and submit insurance authorization requests for patient services. Follow up with insurance companies to ensure approvals are obtained promptly, and address any issues or discrepancies that may arise during the authorization process.Manage the scanning and faxing of important medical documents, including physician orders, insurance authorizations, and patient records. Ensure that all documents are accurately archived in the electronic medical records system for easy access and retrieval.Obtain and document necessary patient-related information for those transferred to an inpatient facility. Assist field clinicians by providing support and ensuring all required documentation is complete and accurate.Actively engage in the organization’s Quality Management program by identifying areas for improvement and implementing strategies to enhance patient care and departmental efficiency.Participate in ongoing personal and professional growth activities to maintain and enhance nursing knowledge, skills, and competencies in home health care practices.Reports issues regarding major responsibilities to the Director and or the Clinical Managers or RN designee for resolution.Provides accurate and timely documentation.Actively participates in Quality Management program.Participates in personal and professional growth and development.Knowledge of state, federal, local and accreditation regulations pertaining to the delivery of home health services.Performs other duties as assigned by the Registered Nurse.

Qualifications:

Current and active California Licensed Vocational Nurse (LVN) license.Minimum one year experience required. Community/home health or medical/surgical experience is preferred.Current CPR certification.Licensed driver with insured automobile - in accordance with state and/or organization requirements.Complies with accepted professional standards and practice.Demonstrates strong verbal and written communication, and organization skills.Strong interpersonal skills with the ability to work with individuals across all levels of the organization.
