Primary Duties
Knowledge of HDMC policies & procedures, nursing certification requirements. Performs job duties within legal and professional regulations and guidelines. Applies nursing principles, techniques and practices according to HDMC policies and procedures. Behaves in a manner to promote a professional and caring environment of care at all times. Demonstrates an understanding of , Department of Healthcare Services, Title XXII, OBRA, and MediCal Subacute Program requirements applicable to Skilled Nursing Facilities (SNF) and MediCal certified Subacute ventilator programs. Performs all skills appropriately with consideration to age specific needs of young adult, middle-aged adult, and older adult residents. Residents are treated with sensitivity and appropriateness in approach to age, culture, religion, gender, and sexual orientation of the residents.Maintains current professional Licensed Vocational Nurse and appropriate Basic Life Support certification and provides facility with a copy of these documents in a timely manner to ensure practice is covered by active license and certification at all times. Keeps up to date educationally, especially in areas of Specialty. Completes initial and annual competencies and annual health and safety requirements timely. Orients new LVNs as assigned. Assists in appropriately orienting registry and float staff as assigned by the Charge Nurse. Is helpful, positive and supportive when working with registry and float staff. Knowledge of Residents’ Bill of Rights and Elder Abuse reporting regulations. Observes Residents’ Bill of Rights and reports incidents of elder abuse, if indicated. Carries out duties in a manner sensitive to and appropriate to age (primarily adults and older adults), culture, religion, gender, and sexual orientation of the residents. Controls the environment to prevent hazards and cross infection by observing HDMC policies and procedures. Arrives and leaves work timely. Follows HDMC Human Resources policies, especially regarding dress code, call off policy, breaks, lateness and sleeping while on duty. Communicates with Charge Nurse, other team leaders and Certified Nursing Assistant (CNA) staff, residents, family members and visitors, to ensure a professional two-way flow of information regarding resident care and other issues. Assesses residents daily and on an as-needed basis, and communicates concerns to the Charge Nurse using sound nursing judgment and decision-making skills. Communicates regularly with the Director of Nursing or designee regarding resident care and other issues not resolved by discussion with the Charge Nurse.Helps ensure department operates in a financially sound manner by using supplies and resources appropriately. Other duties as assigned. Carries out on a daily basis, and/or as needed:Administers and documents ordered medications and treatment according to standards of practice for Skilled Nursing FacilitiesMaintains contents of medication carts and orders individual and floor stock supplies as needed.Follow regulations regarding locking of medication carts and labeling/dating of medication bottles.Follow correct protocol with regard for administration and documentation of controlled substances.Completes weekly summary on residents and updates care plan as needed.Only LVN with Phlebotomy/IV certification start IVs and obtain blood samplesCompletes assigned admission documentation and assessmentsCommunicates need for equipment or supplies to Charge Nurse or Unit SecretaryParticipates in Performance Improvement and Performance Improvement Training team as requestedComplies and ensure others comply with all resident privacy and HIPAA requirementsParticipates in monthly recapitulation process as requestedEnsures CNAs complete assigned resident care tasksProvides resident and family education and training as assignedEnsure DPAC/Advance Directive wishes carried out in accordance with HDMC policy and procedureCompletes Incident and Opportunity to Improve formsCompletes assigned discharge and other documentationEnsures body fluids for ordered test are obtained timely.Ensures if injured reports and obtains treatment as needed per HDMC policyQualifications
Licensed Vocational Nurse (LVN) BLS Certification1 year of previous experience in Subacute Ventilator Unit, Acute Hospital experience or an acquired equivalent competency in caring for people in a Subacute Ventilator Unit settingIV/Phlebotomy certified preferred#LI-JS4
