About the Role: We’re seeking a Licensed Vocational Nurse (LVN) to join our client’s team in a unique care coordination position focused on ensuring foster youth have access to necessary psychiatric medication and support services in Santa Rosa CA.

Schedule:

Monday – Friday, 8:00 AM – 5:00 PM Flexible start/end times available 40 hours per week No weekends required

Key Responsibilities:

Coordinate with psychiatrists and child welfare staff to track prescribed psychiatric medications for foster youth Generate and maintain state-required reports on psychiatric medications Collaborate with social workers and therapists to coordinate psychiatric appointments Ensure complete and accurate documentation for medication prescribing Facilitate care transitions when youth change living situations Maintain continuous communication between service providers and systems

Required Qualifications:

Active California LVN license Strong organizational and data tracking abilities Excellent communication skills Ability to work independently and as part of a team

Preferred Qualifications:

Experience with or knowledge of the foster care system Background in psychiatric care or medication management Experience in care coordination
