Responsible for assisting physicians/providers with patient care, rooming patients, clerical, environmental and organizational tasks. Responsible for appointment scheduling and chart management. Provides information to patients so they may fully utilize and benefit from the practice services. Conveys a positive image of the practice. The MA II, as a result of experience, training and proficiency in the department, is able to perform all requirements of the position at a competent level. As a Patient Navigator guides patients through the healthcare system and ensures that patients and members are able to access culturally and linguistically appropriate services in a timely and cost effective manner. The Patient Navigator must be knowledgeable about community resources, including financial, educational, social and emotional support services available to patients. The Patient Navigator shall demonstrate cultural competency with respect to populations served and will track patients through completion of all diagnostic and treatment care plans to ensure timely receipt of care. The Patient Navigator is responsible for coordinating a wide range of disease registry activities and running routine reports to facilitate patient tracking. Works closely with the patient care team and key collaboratives across YNHHS, NEMG staff. PM staff, hospital based Care Coordinators and Community based resources. The patient care team may include home health and other hospitals/facilities outside the Yale New Haven Health System EEO/AA/Disability/Veteran Responsibilities 1. Performs patient care responsibilities as assigned. 1.1 Identifies self to patient/family and explains to patient/family the roles of each member of the care delivery team. 2. Performs administrative responsibilities as assigned. 2.1 Sends/receives patient medical records and abstracts new patient records into EPIC. 3. Performs environmental responsibilities as assigned. 3.1 Sets up instruments and equipment according to department protocol. 4. Performs other related organizational duties as assigned 4.1 Maintains patient autonomy, confidentiality and dignity. 5. In addition to the Medical Assistant II responsibilities will also perform the following Patient Navigator responsibilities. 5.1 Demonstrate and apply knowledge of the philosophy/principles of comprehensive, patient-centered, developmentally appropriate, culturally sensitive care coordination services 6. Performs other duties as needed and as assigned. Qualifications EDUCATION High school graduate or equivalent. EXPERIENCE Certification/Certification eligible Medical Assistant or two years? experience as unit secretary medical secretary or related field, LICENSURE Certification/Certification eligible Medical Assistant preferred SPECIAL SKILLS Excellent, organizational, communication, customer service and teamwork skills required. Ability to multi-task is essential. EMR experience preferred and working knowledge of Microsoft office applications required. CPR certification preferred. Bilingual preferred but not required. Demonstrates professional, appropriate, effective and tactful written, verbal and nonverbal communication with patients, families, medical staff, colleagues, vendors, and other departments throughout the continuum of care to promote continuity of care and services and enhance clinical image. PHYSICAL DEMAND MMR is required. Varivax (chicken pox vaccine) or evidence of prior chicken pox is required. Hepatitis B (or signed declination) is required for those with potential exposure to blood/body fluids. Tdap and influenza vaccination are strongly encouraged. Must complete work in a timely fashion.
