Manager, Clinical Care Transformation

The Manager, Clinical Care Transformation provides daily oversight of the care pathway development team, community readmission prevention team, several USN&WR initiatives, zero harm initiatives, and works in association with all members of the Clinical Care Transformation team which includes Clinical Documentation Improvement (CDI); Hospital Quality Improvement; Data Analytics and Performance Improvement; Data Science and Process improvement and Medical Staff Services.  The Manager, Clinical Care Transformation collaborates with the entire healthcare team to ensure reporting accuracy and timely submission of data to various stakeholders.  The incumbent will manage the design, planning, and production of dashboards and analysis to support clinical excellence, and quality improvement initiatives.

Manages all aspects of clinical quality data of focus and meets established deadlines, inclusive of data collection, data analysis and interpretation, data submission, daily quality reporting issues, and outcomes report review.

Utilizes the Electronic Health Record (EHR) and any other appropriate resources to obtain valid, reliable data for developing dashboards and scorecards.

Performs data collection, data entry, abstraction, validation, and analysis. Prepares and submits timely quarterly data harvests to appropriate data warehouses.

Interprets and translates data for senior management to clearly understand analysis and use it for meaningful decision making, and strategic planning purposes.

Assumes facilitation role with clinical service line and quality teams to identify and verify patients for inclusion, identify and verify criteria and protocols for inclusion/exclusion, obtain clinical interpretation of data elements, and to support clinical improvement initiatives.

Manages entire process of clinical care pathways which includes research, scheduling and attending meetings with multidisciplinary teams, data collection, submission, validation, and audit.

Collaborates with providers, clinicians, and other professionals to ensure compliance with documentation

Works with team members of Quality Data and Analytics Team and Clinical IT Teams to incorporate clinical data into the Electronic Health Record and develop/ validate reports.

Uses clinical databases and national benchmarking to continuously identify opportunities for improved outcomes and cost reductions, and relay findings and recommendations to appropriate decision maker.  

Evaluates and analyzes opportunities to merge data elements from various electronic medical record sources.

Monitors quality performance, outcomes and/or safety metrics to make recommendations for targeted process improvement projects. Prepares briefings and reports.

Follows up and follows through on assigned action items for various projects, meetings, and committees. Solves data integrity issues as they arise, and monitor the addition of new data, analytics and reporting systems.

Establishes and continuously assesses the effectiveness of the internal controls within the unit and compliance with University policies and procedures. Ensures employees are trained on controls within the function and on University policy and procedures.

MINIMUM QUALIFICATIONS                                                                                                              

Education:

Bachelor's degree in relevant field

Experience:

Minimum 3 years of relevant experience
