Job Details:

The Clinical Pharmacy Manager provides comprehensive pharmaceutical care to home infusion and specialty patients of all ages through medication order review, preparation, dispensing, documentation, monitoring, patient counseling, and medication reconciliation. Carry out day-to-day duties and responsibilities as they pertain to providing safe and effective Home Infusion therapies. Act as a resource to clinical pharmacy staff.

Responsibilities

All responsibilities of the pharmacist role.

Serves as a patient advocate for all medication-related issues. Proactively seeks to optimize patient outcomes through evidence-based, patient-specific pharmacotherapy. Considers all possibilities.

Utilizes the biomedical literature to provide drug information to patients and other healthcare providers. Questions can include, but are not limited to, drug dosage as it relates to medical condition, drug-drug compatibilities, extended stability, USP <797>/<800> compliance, and first-dosing eligibility.

Represents the organization and partners as a member of the clinical leadership team

Participates in departmental activities, staff meetings, system initiatives, quality improvement programs, and other projects as required or assigned. Routinely facilitates high-level discussion and best practice across the organization.

Maintains a well-rounded knowledge of all patient populations and facets of pharmacy practice while concentrating on areas of specialty.

Monitors the medication pipeline and anticipates changes in medication therapy and the potential impact it will have on the clinical and financial delivery of patient care.

Participates in departmental activities, staff meetings, system initiatives, quality improvement programs, and other projects as required or assigned. Routinely facilitates high-level discussion and best practice across the organization.

Coordinate daily work schedules for pharmacists, interns, and PCC’s.  Oversee and instruct clinical pharmacists and pharmacy students within specified team on an ongoing basis.

Assist the physician and other healthcare professionals involved in planning and implementing patient care in the home to ensure safe and effective drug therapy, setting therapeutic objectives for drug therapy, and establishing parameters for monitoring the safety and efficacy of drug therapy.

Attend regular staff meetings to gather and disseminate information.

Establish positive working relationships with both internal and external customers. Evaluating partner requests, adjustments to workflow, and implement, as appropriate.

Participate in the development and presentation of orientation, education and training programs to other staff members fostering a multidisciplinary team environment.

Work with the pharmacy manager of compliance to apply for state licensure for appropriate pharmacies. Assist with compiling all pertinent information, documentation, signatures etc. File documentation as appropriate.

Assist leadership with other projects as appropriate.

Works with the team to support technology by creating and implementing programs and workflows for both internal and external partners.

Act as a resource for clinical pharmacy staff.

Perform essential duties of the clinical supervisor in his/her absence.

Understand and adhere to all applicable State, local and federal laws and /or regulations, company policies and procedures, external accreditation / review standards.

Performs other duties as assigned.

Skills & Abilities

Excellent research and verbal/communication skills.

Excellent organizational skills

Proficient in Microsoft office suite

Requirements

B.S./PharmD degree from an accredited School of Pharmacy.

2-3 years pharmacy experience in an alternate setting/home infusion, preferred

Willingness to obtain licenses in multiple states.
