As part of our commitment to 1 in 4 patients on Home, DaVita is significantly expanding the level of support to field operations for Home growth.  

As a member of the Home National Team, the Manager, Home Modalities is responsible for supporting home growth across DaVita, specifically as it relates to ICHD to Home conversions. This teammate will work collaboratively with field leaders to drive implementation of new programs and the adoption of existing programs. They will coach teammates, monitor performance, improve processes, and generate measurable outcomes. 

Serve as a subject matter expert and consultant to field teams on best demonstrated practices

Collaborate with local leadership across multiple Palmers (including Regional Operations Directors, Home Division Leads, Facility Administrators and Regional Home Managers) to remove patient barriers to selecting a or making it to their modality choice.Coach teammates to effectively coordinate and communicate with the clinic interdisciplinary team to plan education delivery and transition steps, when appropriateLead the recruiting, training, and onboarding process for teammates in partnership with local leadership teams.Virtually attend management calls, facility/region meetings, patient/teammate education sessions, patient/teammate trainings to provide insights and coaching for continuous improvement.

Monitor, track, trend, and report key metrics including but not limited to effectiveness, efficiency, patient modality choice, and other key drivers. 

Drive high productivity and effectiveness from the facilities, regions, and divisions they support. Ensure robust tracking, schedule optimization, and regular assessment of resultsRegularly deliver feedback, assessment and training to teammates based on observation and metrics review.Develop clear & concise action plans in high-opportunity areas in partnership with local leadership. Follow-up to ensure forward progress to promote continuous improvement toward goals.

Identify and escalate opportunity areas to field & Home National Team leadership. Provide consultancy and partnership to overcome obstacles and problem solve. 

Perform other duties and responsibilities as assigned including but not limited to attend team meetings, phone conferences, and training as needed to know, understand, and follow teammate guidelines, employment policies, and department or company procedures

Salary/ Wage Range

$82,800.00 - $121,600.00 / year

Compensation for the role will depend on a number of factors, including a candidate’s qualifications, skills, competencies and experience and may fall outside of the range shown.
