Job Description: A Brief OverviewThe Manager provides each resident with professional nursing care, assessments and evaluations. Is responsible for the management of nursing activities performed by household staff ensuring that the highest degree of quality of care is maintained at all times. The Manager is shared between two 16-bed households, working with a team of cross-trained staff to coordinate the functions of the households.What you will do + Leadership Functions • Maintains a sense of “welcoming” through elimination of institutional barriers • Assures that all people involved are heard. • Engages resident, staff, and families in the growing relationships of the community. • Facilitates conflict management while respecting diversity of people, their strengths and ideas. • Practices highly sensitive interpersonal and communication skills with staff, families, physicians, other departments, and residents. • Acts as a coach to keep the processes resident-centered. • Guides household staff in meeting needs of the residents, practicing routines based on their wants and desires. • Models the way in the household. + Medication & Treatment Administration Function • Ensures that all medications/treatments given are administered safely and accurately in accordance with the physician’s order. • Ensures the adherence to the facility guidelines governing the administration of medications. • Completes medication error form as required. Participates in performance improvement activities related to medication errors as requested. • Monitors residents for adverse events and reports appropriately. • Follows the facility procedure for allowing residents to self-administer their medications. Completes appropriate initial assessment and periodic review to ensure that the resident is capable of self-medication. • Monitors use of Psychotropic medication use per guidelines in the household. • Keeps MD informed of the status of behavior problems and effectiveness of the medications used. • Works with staff to strategically time medications/treatments to coincide with resident's individual routine. + Personal Care Functions • Admits, discharges and transfers residents to and from the household. • Arranges for physician visits, provides information to the physician regarding the resident and initiates new orders. • Keeps the family informed of resident's condition. • Observes nursing care and visits with the resident/families regularly to ensure that quality nursing care is carried out according to plan of care and resident/family choice. • Identifies and resolves care related problems or concerns of residents, family or staff. + Care Planning Function • Collaborates with the Care Plan Facilitator on the Resident Assessment Instrument (RAI) including MDS, RAPs, Triggers, and Care Plans for assigned household residents, as dictated by Facility, State and Federal Guidelines. • Ensures timely planning, scheduling, and revising of the RAI process. • Involves resident/family/staff in data collection whenever possible. • Incorporates assessment data into the care plan. • Coordinates care conferences and multidisciplinary care planning process initially, quarterly, and with significant change. • Initiates the care plan, states problems clearly with measurable goals and ensures that approaches are reflective of the current status of the resident. • Ensures implementation of the care plan for each individual resident in the household. • Monitors care plans periodically to assure they always reflect current status of the resident. • Evaluates resident progress towards goal accomplishment and update plan accordingly. + Documentation Functions • Completes assessment on all new residents within 24 hours of admission. • Completes quarterly summary of resident status. • Follows facility guidelines for all documentation • Documents interventions and responses accurately and completely. • Processes and transcribes physician’s orders and implements according to guidelines. • Charts all reports of incidents involving residents. Notifies physician and family as required. • Completes admission, transfer, and other required forms in accordance with established procedures. + Personnel Functions • Contributes to and consults on all aspects of household personnel management: hiring, training, and mentoring. • Shares information appropriately with household staff. Gives clear directions. • Assists in ensuring that the licensed nurse and nursing assistant are qualified to perform the procedure involving nursing care. • Functions within the scope of the Nurse Practice Act, utilizing recognized standards of clinical practice, as defined by the state of Iowa. • Uses good judgment when replacing staff due to illness/absence. • Identifies staff performance issues and provides feedback through coaching, counseling, performance evaluations, and discipline. + Performance Improvement Functions • Recognizes problems and assures quality is obtained and maintained. • Provides data for performance improvement teams and long term care as requested. + Education Functions • Independently seeks educational opportunities through workshops, in-services, and professional journals. • Shares knowledge and skills through participation in in-services, orientation of new staff, day-to-day interactions with staff and educational sessions. • Identifies staff training needs. Qualifications + Graduate of an accredited school of nursing Required + Associate’s degree (AA) In Nursing (ADN) Required + Bachelor’s degree in arts/sciences (BA/BS) In Nursing (BSN) Preferred + 4-6 years Health care experience within the past five years Preferred + Minimum of 5 years' experience in nursing practice, prefer leadership experience Required + 1-3 years' Experience with Microsoft Suite (Word, Excel, PowerPoint, Outlook, and Teams) Preferred + Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction. + Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar. + Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation. + Being aware of others' reactions and understanding why they react as they do. + Actively looking for ways to help people. + Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times. + Monitoring/Assessing performance of yourself, other individuals, or organizations to make improvements or take corrective action. + Adjusting actions in relation to others' actions. + Using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems. + The ability to see details at close range (within a few feet of the observer). + The ability to communicate information and ideas in speaking so others will understand. + The ability to tell when something is wrong or is likely to go wrong. + The ability to listen to and understand information and ideas presented through spoken words and sentences. + The ability to identify and understand the speech of another person. + The ability to exert maximum muscle force to lift, push, pull, or carry objects. + Registered Nurse Upon Hire Required + Basic Cardiac Life Support & First Aid - American Heart Association within 60 Days Required + Dependent Adult Abuse Mandatory Reporter - Iowa Department of Human Services within 180 Days Required Qualifications: RN Manager for Skilled Nursing Facility very involved in process of frequent admissions and discharges. RN Manager of skilled unit must be competent in skills such as IV insertion and maintenance, tube feedings, wound dressings, IV ports and dressings, drains, and orthopedic equipment. Skilled Nursing Manager may need to assist on floor PRN.RN Manager for Memory Care (ICF) is expected to provide guidance in managing aggressive patient behaviors through de-escalation techniques, staff training, and ensuring compliance with care plans and safety protocols.All long-term care RN Managers are expected to lead and support the nursing team in delivering compassionate, patient-centered care while maintaining a safe and therapeutic environment. Additionally, fostering teamwork and promoting professional development among staff is a critical aspect of this role.If any RN Manager is out of the office, it is the expectation for other RN Managers to assist and provide coverage for other households per department needs.
