Avalon Springs is hiring a Social Services Manager to support their team in Mercer, PA. This is a daylight position.

Direct, develop, implement, and evaluate the Social Services functions in accordance with current applicable federal, state and local standards, guidelines and regulations, and internal established policies and procedures; to assure that the medically related emotional, social and spiritual needs of the resident are met/maintained on an individual basis; and to act as a resident advocate for their needs and services.

Responsibilities:

Assist in developing, administering department policies and procedures in accordance with federal, state and local standards, review policies and procedures regularly. Work with Resident Rights Committees as part of the Quality Improvement Program. Provide support counseling and work with emotional problems including assisting resident/family with anxieties and stress caused by illness and admission to the facility, difficulties in coping with residual physical disabilities, fears related to helplessness and death, and the need for institutional and specialized care. Follow the chain of command guidelines for inter-facility problem solving. Assist in the development of department budget and oversees expenditures for compliance within the budget. Exhibit leadership by example in areas of attitude, attendance, hospitality and a sincere interest in quality; treats coworkers, staff and residents with dignity and respect; represent the facility in a professional manner. Provide services to assist residents in maintaining or improving their ability to manage their social, emotional and spiritual needs. Participate in community planning related to the interests of the facility and the services and needs of the resident and family. Assist in obtaining resources from community social health and welfare agencies to meet resident’s needs. Demonstrate thorough working knowledge of and maintains strict adherence to current federal, state and local regulations and internal policies and procedures as they pertain to Social Services. Observe and follow appropriate procedures concerning the facility’s corporate compliance program. Complete portions of the MDS (Minimum Data Set) following assessment of resident needs. Facilitate Resident Council monthly meetings. Develop and implement the social service programs to provide for the psycho-social and spiritual welfare of the residents. Facilitate interdisciplinary team meetings. Handle Resident Grievance Reports and maintain resident complaint log with appropriate follow-up and resolution. Participate as an active member of the SCS Management Team/UPMC-NW Management Team. Supervise social service caseworker and handle any related personnel issues, including performance evaluations. Keep Administrator apprised of any problems or concerns within Social Services. Accept responsibility and accountability for assigned workload, act as a manager, leader and role model in the Social Service Department. Participate in Restraint Committee. Attend interdisciplinary team meetings. Develop and maintain an excellent working relationship with the medical staff, interdepartmental personnel and social agencies to ensure that social service programs can be properly maintained to meet the needs of the residents. Work in coordination with nursing staff to monitor and review drug therapies and behavior interventions and their appropriateness in treatment to utilize the least restrictive option of care while ensuring resident safety. Assess mental health diagnosis and appropriateness of treatment, consistent with OBRA and UMR regulations. Facilitate resident admissions/transfers/discharges and referral process. Maintain confidentiality in regard to resident and employee information and records.
