This is a regional role supporting the spine service line across the South Region, with a primary focus on enhancing and elevating the hospital based surgical spine programs. The Regional Spine Program Manager will achieve this aim by: - Partnering with the physician leaders from both neurosurgery and orthopedics as well as hospital and CTS leadership to develop and pursue plans to improve spine care delivery. - Organizing and co-leading the multi-disciplinary teams that support spine care to pursue and maintain Joint Commission Disease Specific Certification for Spine at the agreed upon hospitals. - Assisting with the development of key competencies and programming to support comprehensive spine care spanning the full continuum of ambulatory and hospital based care.

Responsibilities

A day in the life of a Manager, Spine Program, South Region at Hackensack Meridian Health includes:

 

Assume responsibility for administering and maintaining the existing Spine DSC. In partnership with physician leadership, co-lead the efforts to pursue Spine DSCs at additional agreed upon sites. Assume oversight and on-going leadership for any additional Spine DSCs achieved.Responsible for program data management including but not limited to: formal data tracking and submission for DSCs; development and maintenance of performance dashboards or scorecards to support management of the spine programs.Collaborate with the total joint program clinical managers to leverage programming, protocols, and capabilities in place for the total joint patients and extend it to spine patients where appropriate. Cross cover joint program clinical managers when and where appropriate. Facilitate the development of tools and resources to support adherence to protocols, including order sets and/or Epic clinical pathways or care paths for spine patients, in an effort to create a single standard of care across HMH spine programs in the Region.Assist in the development of the full range of patient engagement and navigation capabilities pre and post operatively including but not limited to: patient and family education, patient reported outcomes, and post-operative follow up.Assist in the development of capabilities (e.g., intake and triage for patients with back pain) and programming that spans ambulatory, non-operative care for spine patients as well as surgical care to facilitate the launching more comprehensive spine care delivery in the Region.Lead process improvement efforts to address gaps and align with evidence based, standardized care. Support the spine programs participation in a national spine registry and partner with hospital, quality, physician, and CTS leadership to develop quality monitoring, reporting and improvement. Other duties and/or projects as assigned. Adheres to HMH Organizational competencies and standards of behavior.

Qualifications

Education, Knowledge, Skills and Abilities Required:

Bachelor's Degree. Clinical background in either nursing or physical therapy and rehabilitation.  Five years of relevant experience - program coordination or quality/process improvement. Data management experience. Effective written and verbal communication skills. Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite programs.  Demonstrates clinical expertise and utilizes advanced assessment skills in areas of clinical specialty to propose intervention and/or action plans utilizing evidence-based research to achieve positive outcomes for patients and families. Analyzes patterns of variance in practice making recommendations for evidence-based changes to improve patient outcomes utilizing a multidisciplinary approach.Organize teams and coordinate meetings and forums to progress initiatives. Manage calendars, timelines, and project plans to drive program improvement. Evaluates protocols and standards of care in light of current research findings related to specialty areas. Utilizes and disseminates relevant research findings through clinical practice, education or consultation.

Education, Knowledge, Skills and Abilities Preferred:

Bachelor's Degree in nursing or allied health. Advanced degree.Advanced Practice Nurse / Nurse Practitioner. Direct experience in the spine, neurosciences, or orthopedic service lines.
