Manages the day-to-day operation and coordination of Day Center activities. Responsible for recruitment, staffing and supervision of personnel assigned to the Day Center including direct supervision of aides performing personal care activities; may facilitate morning meeting, team reassessments and family meetings in conjunction/in absence of the Director of Site Operations; acts as a liaison for Day Center participants to communicate concerns/acute issues to clinic.

Responsibilities

Job duties specific to this position:

Directs and supervises the daily work of staff, volunteers and contract personnel in the Day Center; supervises paraprofessional caregivers in participant related activities/care activities. May assist Director of Site Operations with organization and facilitation of multidisciplinary team meetings. Determines the number, type of staff, and supervision required to meet the clients’ needs and ensures adequate coverage is in place during hours participants are in the Day Center; selects and orients staff for the Day Centers responsibilities. Establishes and maintains cooperative working relationships among staff. Evaluates participants’ changing needs and makes necessary adjustments with other team members input to ensure that proper care is provided in the Day Center as required; reports acute issues of Day Center participants to clinic personnel.Develops an individualized day center plan of care for new enrollees as appropriate. Maintains and updates the plan of care every 6 months, annually, and as needed. May facilitates morning meeting, initial care plan meetings, reassessments and discharge/transfer meetings in absence of Director of Site Operations. Participates in family meetings, nursing home care planning meetings, and clinical case conferences as appropriate.Coordinates transportation with transportation director to meet the needs of participants. Is responsible for the inventory and ordering of supplies for Day Center; ensures participants receive necessary personal care supplies, supplements, etc. Participates in daily program activities as needed; maintains a safe, structured environment for participants, staff, volunteers and visitors within the Day Center.Collects and submits data required for management and regulatory oversight. Assists in development and annual review of policies and procedures specific to Day Center. Prepares and submits statistical reports required by the Director of Site Operations or other administrative staff.Greets potential new enrollees on visit day, provides a tour, and assists with coordination throughout the visit day including arranging for seat assignment for the noon meal, which may include visitors and family.Is actively involved in the QAPI Program; assists as requested in data collection and development of action plans to monitor, assess and continually improve care, program outcomes, and customer satisfaction; actively works on task force(s) as requested.In collaboration with Education Services, coordinates in-service training for Day Center staff and volunteers; conducts monthly staff meetings to provide and promote productive two-way communication.Regularly attends Administration Meeting. Reports on projects, department and policy changes, etc. to promote communication among management team.Completes other duties required to meet the goals and objectives of the PACE program.Practices safety awareness, reports any possible safety/environment issues appropriately, and adheres to safety policies and procedures.Demonstrates adherence to all compliance policies and procedures. Is responsible for promoting and fostering compliance in the workplace.Ensures the highest quality of care by performing responsibilities according to the highest professional standards.Demonstrates knowledge of PACE/D&TC Emergency Preparedness Plan and can verbalize their role during an event.

With the Director and Human Resources guidance, makes or approves recommendations on employee hires, transfers, promotions, salary changes, disciplinary actions, terminations and similar actions. In conjunction with HR, resolves grievances and other personnel problems in a fair, timely and consistent manner.May have input to or recommendations for operating budgets.

Qualifications At least two (2) years previous experience in providing service to LTC or frail elderly clients in a health facility or community based setting.Degree or license in a health related field necessary; prior management experience preferred.Demonstrated ability to plan, organize and direct work of others.Exceptional verbal and written communication skills.Must demonstrate ability to perform duties with minimal degree of managerial oversight.Proficient in MS Office tools, particularly MS Word and Excel, and able to learn new software rapidly.

An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.
