Job Requirements:

The Case Management Manager helps facilitate the management of the case management and admission coordination departments in collaboration with Department Director.The Manager oversees a staff of RN’s and social workers responsible for patient care coordination and patient flow.Implements the developed case management programs including utilization review and intake or discharge planning.The Manager helps facilitate the management of the area through the direction of human and physical resources adjusting the daily operations to meet the changing needs/environment of the departments.The Manager strives for excellence in patient care and patient care delivery by building a collaborative work environment among all staff and departments.The Manager supports an environment of healing and wellness and is an exemplar for compassion, acceptance, respect, and empathy. May cover cases as needed.

Job Requirements:

Bachelor’s degree in nursingKy RN licenseThree years’ experience in acute careMust obtain ANCC, CCM, ACM-RN within 2 years

Preferred Requirements:

Master’s degree in nursingCase management certification (ANCC, CCM, ACM-RN
