Manager Home Health

This position is a licensed RN, PT, OT or SLP with the skills and abilities to perform home health staff RN or Rehab duties. The Home Health Clinical Manager serves as a communication hub responsible for coordinating the multidisciplinary home health services for the clinical team. This position acts as a liaison between Access Data Entry, Scheduling, Quality and clinical Staff. Manages multi-discipline Clinical Team Caregiver performance and productivity. This position provides communication, information and the tools necessary to maintain best practices in all team roles. The Home Health Clinical Manager will have access to confidential organizational financial data pertinent to OASIS data integrity in order to compile data and/or reports utilizing both workflow and financial planning information.

Required Qualifications: 

Bachelor's Degree in Health care related field. National Provider Identifier (NPI) number within 30 days of hire. Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request.  3 years in the nursing or rehab field. This position requires that you obtain an NPI number within 30 days of starting in the position. The NPI process is free, and the application may take several weeks to process. Preferred Qualifications:
