RN License with BSN is required Primarily off-site work time however local residency required for occasional visits to the Medical Group Practices across our entire region **Position Purpose** The Clinically Integrated Network (CIN) Manager of Care Management will direct and oversee the day-to-day operations for population health and the care coordination team, and helps to develop the population health programming needed to effectively manage utilization and total cost of care.
