Primarily off-site work time however local residency required for occasional visits to the Medical Group Practices across our entire region
**Position Purpose**
The Clinically Integrated Network (CIN) Manager of Care Management will direct and oversee the day-to-day operations for population health and the care coordination team, and helps to develop the population health programming needed to effectively manage utilization and total cost of care. Has direct oversight of the clinical staff (RNs and LPNs), as well as any additional support staff working across the CIN population health programs.
**What you will do**
+ Assist in developing and implementing the population health programs required to achieve success in the Medicare Shared Savings Program (MSSP) and other value-based care programs
+ Assist in determining utilization and cost benchmarks, goals, and/or qualitative outcomes for the care management programs
+ Establish criteria for care management program enrollment, transition, and discharge
+ Recruit skilled staff members, develop effective workflows and processes, and train staff
+ Collaborate with quality and performance improvement team to ensure alignment in priority quality metrics
**Minimum Qualifications**
+ Current RN License in CT/MA required
+ Bachelor’s degree in nursing required
+ Minimum of three years experience managing a team of healthcare professionals
+ Minimum of three years working within an Accountable Care Organization (ACO) or medical group participating in value-based contracts
