Amergis Healthcare Staffing is seeking a Manager of Complex Care Coordination who is responisible for managing and leading a multidisciplinary care team composed of care coordinators and community health workers to provide effective, efficient care coordination. Leads the day to day activities of the care coordinator’s, including scheduling, case assignment, mentoring and problem solving.
Responsibilities: Maintains full caseload, complete with ongoing individual service plans Assess ongoing changes in behavior, circumstances, or conditions that may affect patient safety Engages /involves client and sponsor in the casework process Prepares, compiles, and submits accurate case files Serves as a liaison with stakeholders (i.e.. legal providers, immigration court) Consults with supervisors to determine decisions as they relate to the safety, well-being and permanency of patient Maintains accurate records, files, forms, statistics and additional relevant information in accord with agency policy, licensing, and/or funding requirement
Minimum Requirements:
