Under the direction of Executive Director Community and Population Health Services, the Manager Population Health is responsible for designing and implementing strategies aimed at improving the health outcomes and wellbeing of the community, focusing on preventive measures, health promotion, and mental health needs that align with the hospital's mission and goals. Leads, supervises, and mentors a team of social workers, mental health clinicians, and community health workers, and other healthcare staff, ensuring professional growth and development.
QualificationsMaster’s Degree in Social Work and licensed by the State of New Jersey as a Licensed Clinical Social Worker required. Five (5) years of post-master’s work experience in Social Work, including three (3) years in an acute care hospital setting, required. Two (2) years of social work supervision required. Experience in healthcare or community health settings, with a focus on population health management. Strong understanding of public health principles, epidemiology, and health behavior theories. Proficiency in data analysis and interpretation, with experience using health information systems and analytics tools. Excellent communication and interpersonal skills, with the ability to engage diverse stakeholders and build collaborative relationships. Proficiency with Microsoft Office Suite required. Experience achieving superior performance in a unionized environment preferred.

PHYSICAL DEMANDS: This position requires sitting, talking, and using of hands > 2/3 of the time; standing, walking, stooping, and crouching <1/3 of the time.
