**Description**
**8am-5pm / Full Time**
We are seeking a capable **Manager, Ambulatory Care Coordination / Emory Healthcare Network,** who has acquired broad experience in caring for patients, and has developed a sound understanding about the care of a particular patient population.
+ Assists Director of Ambulatory Care Coordination in the development and implementation of clinical operations for the care coordinators and patient population management.
+ Responsible for the daily management of the care coordinators work and workflow.
+ Develop and implement processes to improve outcomes and improve metrics.
+ Plan and implement process for data collection for quality metrics for patient population management.
+ Budgetary responsibilities for operations related to care coordinators.
+ Participation with design of medical home concepts and NCQA certification.
+ Ability to analyze data and implement change to improve patient outcomes and quality metrics.
+ Knowledge regarding the concepts for care coordination and management of high risk populations.
**MINIMUM QUALIFICATIONS:**
+ Graduate of an accredited nursing school.
+ Associates degree required, Bachelors degree preferred.
+ Must have a valid, active unencumbered Nursing license or temporary permit approved by the Georgia Licensing Board.
+ Care Management certification preferred.
+ Five years experience preferably in case management, utilization review and/or quality improvement experience in outpatient and inpatient clinical processes.
+ Previous leadership experience required.
