Responsibilities

Facilitates and coordinates the orientation and staff development programs for the case management staff. Participates in development of leadership skills in case management staff. Validates appropriateness of case management through both indirect and direct involvement in the clinical area. Assumes responsibilities in collaboration with the Department of Outcomes Management for operational and personnel activities. Responsible for coordination and facilitation of appropriate functioning of case management committees.

This role is eligible for TN sponsorship.  Successful sponsorship will require state licensure and completion of the VisaScreen or equivalent certification 



Qualifications

Graduate of a Bachelor’s degree in nursing program. 

2 years of hospital-based case management, including discharge planning and utilization review experience preferred. 

Experience with utilization review, including admission and discharge criteria and discharge planning preferred. 

Additional Qualifications:

Knowledge of Centers for Medicare and Medicaid Services (CMS), Medicare, Medicaid, HMO and department of Children and Families, community resources, discharge planning, extended care and rehab facilities and durable medical equipment.

Promotes quaqlity in day to day case management by monitoring, evaluating, plannin coverage for identified case management needs. 

Facilitates case management program, including: policy/procedure revision and development, staff recruitment and retention. 

Coordinates performance improvement activities of case management staff. 

Conducts/factilitates orientation and development ofnew and existing case management staff. 

Knowledge of clnical pathways, resource management, community resources, interviewing techniques, medical terminology, good written and verbal communication skills, leadership skills. 

Well developed communication and organizational skills required, with the ability to problem solve and facilitate change effectively. 

License or Certifications:

Florida RN License is required.
