MAT Clinician - Provisional/LPC, LMFT, LCSW

The role will provide specialty mental health and substance use counseling using evidenced based treatment modalities to patient’s engaged in MAT (Medication Assisted Treatment) services within a primary care setting. Provide services including, but not limited to, assessment, treatment planning, long-term counseling, education, case management, advocacy, and follow-up.

Direct Patient Care

Provide mental health and substance use treatment services to patient’s engaged in Medication Assisted Treatment services within a primary care setting. Coordinate with other staff members and treatment team in a primary care integrated model to assess, treat, and manage patients with mental health and substance use concerns. Team members include primary care providers, MAT providers, Behavioral Health Consultants, CADC’s, RN’s, and other integrated primary care team members. Conduct clinical evaluation, including screening, assessment, and diagnosis of substance use disorders and Co-Occurring Disorders. Perform intake, evaluation, orientation, treatment planning, and counseling while documenting within Epic EHR system. Assess new intakes to determine presence and extent of substance use and co-occurring disorder issues; completes evaluation report. Develop and document individualized treatment plans for substance use and co-occurring disorders that address the needs of the substance use patient population. Refer patients to appropriate treatment modality (e.g. inpatient, intensive out-patient, primary care-based treatment, etc.) based on assessment. Develop aftercare and relapse prevention plans. Consult with primary care providers and MAT team in management of patient care. Refer clients to support groups to assist with sobriety maintenance if applicable. Outreach to patients missing services based on guidelines / reports from patient registry or other sources. Deliver patient care within HRSA services scope and approved locations, encompassing patient homes, community-based sites, and providing home-based care when necessary and in adherence to defined services and safety protocols. Local travel required.

Education/Counseling

Provide mental health counseling to patient’s engaged in MAT treatment using evidence-based treatment modalities. Teach patients coping mechanisms to support mental health and recovery goals. Meet with family members and provide guidance and support when needed. Conduct individual, group, family, and/or couple’s therapy. Develop and facilitate groups and educational programs regarding substance use and mental health. Conduct group classes and group visits to promote skill building, education, and improved health for the patient. Provide training and education to care team members to facilitate diagnosis and treatment of substance use within primary care. May perform clinical supervisory responsibilities for Transitions Counselors/CADC.

Reporting and Administration

Attend and participate in Quality Improvement activities. Document accurately within EMR in a timely fashion per system standards. Identify opportunities for program improvement and QI monitors. Participate in continuing education to continually improve skills and abilities and stay abreast of current technologies/practices and keep licensures up to date. Ensure compliance with legal issues including but not limited to client confidentiality and risk management; ensures compliance with federal, state, and local regulations.

Skills & Knowledge:  Has the competencies necessary to: Identify precipitating events; Gather histories of mental, emotional and physical disabilities, alcohol and drug use, and past mental health services; Assess family, social and work relationships; Conduct a mental status assessment; Document a multiaxial DSM diagnosis; Write and implement or supervise implementation of a treatment plan; Conduct and document a mental health assessment; Provide mental health treatment and rehabilitative services within ones scope of practice; Experience working with adult populations with addictions and complex issues.  Execute evidence-based brief interventions. Ability to make quick and accurate assessments of mental health conditions related to substance use. Interest in gathering, tracking and presenting outcome data. Expertise in trauma-informed, culturally sensitive substance use and mental health treatment. Be knowledgeable of local and state resources. Comfort with electronic medical records and charting. Flexible and able to assess and triage on the spot demands of medical staff.
