As a Social Worker ll at MercyOne, you will apply advanced psychosocial theory and methods to individuals of all ages, as well as couples, families, and groups, with appropriate regard for the complexity of given problems. Responsibilities include providing advanced evaluative, preventive, therapeutic, restorative, and personal care services to meet the complex psychosocial needs of MercyOne patients and their families, enabling them to better utilize medical care resources and achieve optimal health. This may include providing consultative services to rural hospitals, home care agencies, and long-term care facilities. As assigned, provides social work services to patients at any point on the continuum of care, from consultation to post-discharge follow-up. The role will also involve navigating financial advocacy for patients and maintaining industry knowledge on billable services. Interaction with individuals across diverse age groups may also be required.

Essential Functions:  

Obtains social history by interviewing patients/families, reviewing medical records, and consulting with physicians and staff. 

Assesses patient needs with advanced competency in psychosocial evaluation, diagnosis, and intervention. 

Develops and implements psychosocial care plans in collaboration with the healthcare team, monitoring progress. 

Coordinates discharge planning by consulting with the care team, providing options/resources, and making referrals. 

Responds to emergencies by offering immediate counseling and assistance, and referring to community resources as needed. 

Provides psychosocial counseling to support patients and families in adjusting to illness or injury. 

Shares information on available services and makes timely referrals to appropriate agencies. 

Supports patients with financial concerns, including insurance benefits, financial assistance, and advocacy for payment discounts or charity care. 

Assists with advance directives, FMLA, and disability needs. 

Offers clinical and administrative consultation to physicians and healthcare professionals through evaluations, case conferences, and education. 

Maintains professional relationships with community agencies and collaborates through contact and correspondence. 

Documents social work activities in patient records and prepares reports on time. 

Participates in Cancer Center accreditation meetings and committees related to social work. 

Leads problem-solving and contributes to program development. 

 

General Requirements: 

Familiarity with medical terminology. 

Knowledge of clinical interviewing techniques. 

Understanding of social work principles, standards, and ethics. 

Basic knowledge of Medicare, Medicaid, and child/dependent adult laws and regulations desired. 

 

Education: 

Master’s degree in Social Work required. 

Two years of experience in medical social work preferred.
